Handling Electronic Privacy Notice

Purpose:

To provide guidelines on submitting an electronic copy of our Notice of Privacy Practices via the internet, when applicable.

Policy:

When and/or if Smoky Mountain OB/GYN Associates develops and maintains a web site that provides information about our customer services or benefits, we must prominently post our Notice on the web site and make the Notice available through the web site.

When applicable, Smoky Mountain OB/GYN Associates will make available electronic copies of the Notice of Privacy Practices to all patients requesting an electronic version. We will monitor this transmission and if it fails to connect with the patient, a paper copy of the Notice will be provided.

Provision of electronic Notice by Smoky Mountain OB/GYN Associates will satisfy the provision requirements if receipt of the Notice by the patient is documented in the patient’s medical record.

The patient who is the recipient of an electronic Notice retains the right to obtain a paper copy of the Notice from Smoky Mountain OB/GYN Associates upon request.
